Custom Puzzle Order Form

To place an order for a custom puzzle, complete and submit the following form. I'll respond by email if there are any details that need clarification.

If you are providing an image for the puzzle, you may either email it to Puzzles@DougsPuzzles.com or mail it. (Please email me for the address to mail to).

Most orders are shipped within two weeks. If you need something by a specific date, let me know and I'll try to accommodate your request.

Piece Size		How big should the pieces be? A puzzle cut in "standard" size pieces will have about one piece for each square inch of puzzle. "Small" is about half that size. "Large" is about twice standard size. I can make pieces any size you wish. If you can't get the number of pieces desired using one of the three sizes, let me know how many pieces you would like the puzzle to have. Larger pieces are available. Email for a price quote.

Figurals & Word Pieces		Figurals are puzzle pieces cut as objects or silhouettes. A word piece is a piece cut in the shape of a word. For some ideas and examples of figurals and word pieces, view the Figurals Page. If you wish your puzzle to contain either of these, indicate how many, then list or describe them. There is a charge of $4.50 each for figural, and $9.00 per word.

Puzzle Summary		Based on the options selected above, your puzzle's cost and estimated number of pieces is shown at left. The actual number of pieces may be more than shown, but will not be less than the estimate. The basic formula for the puzzle price is $12.00, plus $0.70 per piece. There are some additional charges which may apply. For printing images greater than 10" by 16" there is charge of $8 per square foot. International shipping will be higher. There may be a fee for any photo touch up work required.

Sending an Image by Email

You may send an image as an email attachment. The image will be reduced or enlarged as needed to fit the puzzle size you order. All common image formats can be accommodated, including JPEG, TIFF, BMP, and PSD. Larger images will generally work better. Images should be at least 640 by 480 pixels in size, preferably even larger. If you have questions about the suitability of an image, send it to me and I'll let you know how well it will work.

Sending an Image by US Mail

You may also send photos or drawings by U.S. Mail. Please email me for the address. The original image will be scanned and returned unharmed with your order. However, please do not send images that are valuable or irreplaceable, as I cannot be responsible for their loss beyond reprinting cost. If you are concerned about potential loss or damage to an item, it is best to make a copy and send it rather than the original.

I can make a puzzle directly from a poster or print you already own, or you can select an item from a website such as art.com or allposters.com and have it shipped directly to me.

If you are not completely satisified with your puzzle, return it within 30 days, and your purchase price will be refunded.
